Employees are reminded to review their December and January earning statements to confirm 2018 benefit elections.
Corrections effective on the first of the month following the receipt of a form and will not be retroactive. If you took action during open enrollment and need to make a correction, complete a correction form and fax, scan, email or drop off all pages to the University of Oregon benefits office. The two forms are available online:
- Core and optional benefit correction form: Due by Jan. 31
- Flexible spending account correction form: Due by Feb. 28
If you didn’t take action during open enrollment, you can remove the monthly surcharges for tobacco use and spouse/partner other coverage by completing the core and option benefit correction form. All forms listed above should be submitted by the listed due date to the benefits office.
The Office of Human Resources also is partnering with Cascade Centers, the UO’s employee assistance provider, to offer a winter seminar series for wellness, which will be broadcasted live via webinar to UO Portland and the Oregon Institute of Biology in Charleston. HR is also offering a new campus Weight Watchers session, which will begin Feb. 8. Visit the Weight Watchers’ webpage on the HR website to learn more about the Weight Watchers offering in the UO benefits program. Information was also recently posted in Around the O.
Employees also are encouraged to take advantage of the employee assistance program through Cascade Centers to manage stress. The program helps with concerns such as family, relationships, stress, depression and wellness. More information is available online.
The Affordable Care Act requires employers to issue Health Care Information Forms regarding employees’ health care coverage. Required forms will be mailed to employee homes by March 2, which is the deadline set by the IRS.
The forms are required for informational purposes only and are not necessary to file 2017 taxes.
